SJES CHECKING OF SCHOOL FORMS MISSION ACCOMPLISHED

The preparation and review of school forms to ensure the quality and consistency of student information is one of the crucial tasks completed at the end of every school year. In this regard, San Juan Elementary School (SJES) is consistently ready in compliance to DepEd Memorandum No. 37, s. 2022 or the Resumption of the Implementation of the Policy on Checking of School Forms as Prescribed in DepEd Order No. 11, s. 2018 (Guidelines on the Preparation and Checking of School Forms).


On July 3 and 4, teachers from the Kindergarten and Grade 6 had their school forms reviewed carefully at the Division Level. The activity, which was held at San Juan Elementary School, was facilitated by Ms. Lovely M. Gidayawan, the competent guidance counselor, Ms. Esterlina E. Seguan, M.T. in charge of Grade 6 and Belen R. Dizor MT In-charge of Kinder, under the supervision of the partner-supervisor of the school, Dr. Bradley Goldie K. Loo, Education Program Supervisor in Science with Mr. Michael Rull and the supportive principal, Dr. Lucila G. Artuyo

The Division Checking from Grade 1 to Grade 5 was scheduled on July 6–7 and as expected, their forms were reviewed with the same level of diligence.


The following school forms were checked by the School Checking Committee prior to the Division Checking:

SF 1 - School Register

SF 2 - Daily Attendance

SF 5 - Report on Promotion and Learning Progress and Achievement

SF 9 - Progress Report Card

SF 10 - Learner's Permanent Academic Record


Deepest gratitude was extended to the entire School Checking Committee for the remarkable effort and dedication they displayed while reviewing the school forms. Their meticulous attention to detail and commitment to ensuring accuracy are truly commendable.


To all the advisers who invested their time and energy into neatly and efficiently completing the school forms, their diligence and perseverance in completing the school paperwork have now been rewarded.
